As a new patient you should allow ample time before your first appointment. It is also recommended that your have child’s previous health record sent to our office prior to your visit, or you can bring them with you on the day of appointment.

When requesting your child’s medical records, a Medical Records Request Form must be completed and returned to Metropolitan Pediatrics. Once the completed form has been received, our office will contact the parent or guardian informing them of their cost for processing a copy of their records.
Metropolitan Pediatrics asks that you submit requests for referrals at least five business days prior to your scheduled appointment with your child’s specialist. Please fill out the request as completely as possible. The completed request may be faxed back to us, dropped off at the office, or scanned and emailed back to us. Once the referral is complete, the parent or guardian will be notified by phone.
